PACE Presentation

Kerry Conlon from PACE (Program of All -Inclusive Care for the Elderly) will highlight the benefits of the PACE       program and its many offerings on Wednesday, November 3rd at 10:00 AM in the Activity Room.

The PACE model of care will be discussed, as well as eligibility and the    enrollment process.  Additionally, this     introduction will provide an overview of the Four Pillars of Health Aging at  home – a signature piece compiled by a highly- regarded Occupational Therapist at Harbor Health Elder Service Plan.
